It is a recommended way by Oracle to apply patches onto an Oracle Enterprise Manager Agent by using a patch plan.
When Oracle Enterprise Manager Cloud Control is running in the Offline mode, make sure the patches that you want to apply are already available in Oracle Software Library (Software Library). If Cloud Control is running in Online mode, you can connect to My Oracle Support to download the patches that you want to apply.
In this exercise, OEM 13c is running in offline mode, and two patches have been downloaded and uploaded onto Software Library. One patch is a bundle patch, and another one is the latest OPatch utility.

This is a nice example of how to use the Patch Plans.
I am recently the same method and it worked great.
One question remains. If I check “OPatch Upgrade” in the Deployment Options I’m getting the error “OPatch that is of version 13.8.0.0.0 and platform 206 is not found in the Software Library”
How do I load the newest OPatch into the Software Library?
My OMS runs also in offline-mode.
